SECTION 2A: MEMBERSHIP AGREEMENT – applies to all members

(a) the member is 12 years of age or older and plays at his or her own risk; (b) member between 12-17 years of age must have a parent co-sign the form; (c) the member is solely responsible for his or her acts and omissions; (d) the member is solely liable for the loss of or injury to property he or she brings to the Club facilities; and (e) the member must conduct himself or herself in a sportsman-like manner and in accordance with Club rules at all times, or be subject to having his or her membership revoked without notice or reimbursement.

The member agrees to indemnify and save harmless: The Deshantari and Deshantari Badminton Club and its members from all claims, costs, and liabilities, which may arise as a result of membership in the Deshantari Badminton Club or during use of the Club facilities at Hintonburg Community centre. The member also agrees that: (a) membership is not transferable unless approved by the Badminton Club Executive; (b) food and drink cannot be consumed inside the gymnasium; (c) the member will wear only non-marking court shoes in the gymnasium; (d) this Agreement will be in effect until a discontinuity occurs in your membership in Deshantari Badminton Club; and (e) the member’s signature verifies that all information provided by the member in Section 1 is correct, and confirms that he or she has read all of Section 2 and accepts the conditions of the membership agreement.

(Please note that the Deshantari Badminton membership application form must be completed and signed by each applicant applying for Club membership. An incomplete or unsigned application form cannot be accepted by the Club Executive, and each applicant must use his or her own form. Each family member or spouse, for example, must complete and sign a separate form. All information is kept strictly confidential and used only for Deshantari badminton membership purposes. Please make a copy for your own records.)

SECTION 2B: MEMBERSHIP AGREEMENT – applies to 18 years or older

1. I read the membership agreement, section 2A above and accept it’s conditions;
2. I am solely responsible for my acts, omissions and behaviour;
3. I agree to indemnify and save harmless: (a) the Deshantari and Deshantari Badminton Club and its members, and (b) the Hintonburg Community Centre (Ottawa) and its officers, trustees, employees, and contractors from all claims, costs, and liabilities, which may arise as a result of my membership in the Deshantari Badminton Club or during use of the Club facilities at Hintonburg Community Centre (Ottawa);
4. I promise to conduct myself in a sportsmanlike manner and in accordance with Club rules at all times, or be subject to having my membership revoked without notice or reimbursement;
5. I agree that (a) my membership is not transferable unless approved by the Badminton Club Executive, (b) I may not consume food and drink in the gymnasium, (c) I will wear only non-marking court shoes (no street shoes) in the gymnasium, and (d) my signature verifies that all information I provided in Section 1 is correct, and confirm that I have read all of Section 2 and accept these conditions as my membership agreement with Deshantari Badminton Club

SECTION 2C: MEMBERSHIP AGREEMENT – applies to 12-17 age groups

1. I am between 12 or and 17 years of age, and below is my parent’s signature permitting me to play in this club;
2. I read the membership agreement, section 2A above and accept it’s conditions­;
3. I am responsible for my acts, omissions and behaviour; my parent, signatory below, is my guarantor;
4. We agree to indemnify and save harmless: (a) the Deshantari and Deshantari Badminton Club and its members, and (b) the Hintonburg Community Centre (Ottawa) and its officers, trustees, employees, and contractors from all claims, costs, and liabilities which may arise as a result of my membership in the Deshantari Badminton Club or during use of the Club facilities at Hintonburg Community Centre (Ottawa);
5. I promise to conduct myself in a sportsmanlike manner and in accordance with Club rules at all times, or be subject to having my membership revoked without notice or reimbursement;
6.
